To submit a service request, simply complete this form. It's quick, easy, and doesn't cost you a thing. Here's what the form will ask you to do:
1. TELL US WHAT YOU NEED
We'll first ask you what service you need. You can either select a service from our list or from our most commonly requested. Once done, simply click 'Next'.
As you progress, we'll use the information you provide to filter out providers - look at the progress bar at the top of the page to keep track of the providers that match your request.
We'll then ask you where you need your service, so we connect you with providers in your area! You can also opt in to include telehealth if appropriate. For example, if you're looking for occupational therapy, telehealth might be worth exploring - so you avoid long waitlists.
2. TELL US WHO NEEDS THE SERVICE
Managing an NDIS Plan is a team effort, so we know that a whole lot of people are looking for services, whether that be a participant, their family members (such as mum or dad), or a support coordinator or local area coordinator. If you're submitting a request on behalf of someone, we'll ask you for fewer details.
We ask a few questions to ensure we find the right provider. For example, not all providers support children (so if you're a parent looking for support, we exclude those providers from the results!) and unregistered providers can't support agency managed participants. You might know a lot of this, but it's worth explaining why we ask you these questions.
We'll then ask you to verify your phone number, so that we know you're a real person (we know you are, but AI is sneaky!) and let you share any other details that might be relevant.
4. ADD OPTIONAL DETAILS
You can then provide any additional information if you wish - things like how often you need a service or when you want it to start. This can save some additional questions if you choose to progress further with this provider. It can also help a provider determine if they're the right fit for you - but offering this information is purely optional. Remember: You need to click Save at the bottom of the screen to progress to the last screen.
5. CHECK YOUR DASHBOARD
After adding any additional information you can check your dashboard and watch the progress of the request in realtime. As providers begin accepting your request, we'll share their profiles with you in the three tiles, including any reviews they might have. You can then gauge if a provider is going to be suitable! Sit back, relax, and wait for them to contact you.
Please note: You can only submit one service request at a time. If you need to request another service, complete the form a second time. For instance, you might need a physio and a gardener, so complete the form once for each.
